The first step to receiving books is to fill out an application.

• You can print the application on this web site and mail it to us, or
   
  
• contact us by phone or email and we'll send you an introduction packet by return mail. (If we mail you a packet we'll enclose an audio tape that explains who we are and how we operate.

The application packet includes the following items:

• Application for Free Library Service Form
  Each applicant is asked to get a signature from their doctor, nurse, pastor or another caregiver who can certify that they are reading-disabled. (This certification is required by the copyright laws which allow us to record books for the library.)

• Request Form
  By filling out this form you tell us how you would like your books to be selected and which categories you find most interesting.
  
• Large-print catalog listing of some of our books
  This sample catalog will give you an idea of the types of books that are available.
