How to Apply for Books on TapeThe first step to receiving books is to fill out an application. (The forms are available at the links below or you can call us at 206-243-7377 and we will mail you the application form.)
The application packet includes three items. Please print out and complete the forms, and mail them to us for our records.
- The Application Form. Each applicant is asked to get a signature from their doctor, nurse, pastor or another caregiver who can certify that they are reading-disabled. (This certification is required by the copyright laws which allow us to record books for the library.) The application form is available online in two formats:
* Application form in Word format
* Application form in pdf format- The Book Request Form tells us what kind of books you find most interesting, and whether you want to pick titles or have our Librarian select books for you.
- The sample catalog will give you an idea of the types of books that are available. This large print catalog is available in two formats:
* Sample catalog in Word format
* Sample catalog in pdf format (requires Adobe Reader software)
* Or you can call us at 206-243-7377 if you prefer a catalog in audio tape format.
